Pope John Paul II elevated Hoser to the title of archbishop ad personam in 2005. Three years later, on 24 May 2008, Pope Benedict XVI appointed him to lead the Diocese of Warszawa-Praga. He served in this capacity until 2017. During his tenure, he sat on the Presidium of the Polish Episcopal Conference and led the Team of Experts on Bioethics, where he publicly opposed in vitro fertilisation.

Following the completion of this report, he was named Apostolic Visitor for Medjugorje on 31 May 2018. This appointment kept him involved in international church affairs until his later years.
Controversies and Final Years
Hoser faced significant public criticism in 2013 regarding his handling of a priest convicted of sexual abuse. Simultaneously, he removed Wojciech Lemanski from his position for publicly challenging church stances on anti-Semitism and the handling of abuse allegations. Hoser received the Grand Cross of the Order of Polonia Restituta before his death from COVID-19 in Warsaw on 13 August 2021.
