Early Development and International Debut
Born in Benha on 10 November 1984, Fathy began his professional trajectory at Ismaily SC. He secured the 2001–02 Egyptian Premier League title with the club. His rapid rise saw him become Egypt's youngest international player when he debuted against South Africa in 2001 at age 17. By 2003, he contributed to the Egyptian youth team's victory at the African Youth Championship in Burkina Faso.

In January 2007, Fathy signed a three-and-a-half-year contract with Sheffield United for £700,000. He debuted in the Premier League against Tottenham Hotspur on 10 February 2007. After limited appearances, he moved to Al Ahly in September 2007. Later, he joined Hull City on a six-month loan in January 2013, debuting against Charlton Athletic in February. He also spent time in Qatar with Umm Salal starting in August 2014.
Tenure at Al Ahly and Later Career
Fathy served as a key player for Al Ahly across multiple stints. He returned to the club in 2015 following his time abroad. His domestic trophy haul included numerous Egyptian Premier League titles and CAF Champions League victories. In April 2020, he agreed to join Pyramids FC on a free transfer. He remained with Pyramids FC until concluding his professional career in 2024.
International Milestones
Representing his country consistently, Fathy was a member of the squad that won the Africa Cup of Nations in 2006, 2008, and 2010. During the 2010 tournament, he earned recognition as the fair player of the tournament and was included in the team of the tournament. In 2018, he played in the FIFA World Cup held in Russia.
