Raised in a Punjabi family, Akram attended Government Islamia College in Lahore. His path to professional sport was unconventional; he began as a tape ball cricketer before joining Ludhiana Gymkhana. Beyond his contributions to the Pakistan national team, including the 1992 Cricket World Cup victory and his leadership in the 1999 World Cup final, he played extensively for Lancashire County Cricket Club from 1988 to 1998. During his tenure as captain at Lancashire in 1998, the team secured the NatWest Trophy and the Sunday League.

Widely acknowledged for his precision and pace, Akram holds the world record for most wickets in List A cricket with 881. His ODI record of 502 wickets remains second only to Muttiah Muralitharan. On 30 September 2009, he was inducted into the ICC Cricket Hall of Fame, followed by the Hilal-e-Imtiaz award in 2019.

Following his retirement, Akram moved into coaching and administrative roles. He served as the bowling coach for the Kolkata Knight Riders and held positions with Islamabad United and the Karachi Kings.
