For her portrayal, she earned the Academy Award for Best Supporting Actress, becoming the youngest person at that time to receive an Oscar in a competitive category. From 1963 to 1966, Duke starred in The Patty Duke Show, where she portrayed identical cousins Cathy and Patty Lane. Following the conclusion of the series, she transitioned to adult roles, including Neely O'Hara in the 1967 film Valley of the Dolls and Natalie Miller in the 1969 film Me, Natalie. The latter performance secured her a Golden Globe Award for Best Actress in a Motion Picture – Musical or Comedy. Her work in the 1970 television film My Sweet Charlie earned her a Primetime Emmy Award for Outstanding Lead Actress in a Miniseries or a Movie.

Duke served as the president of the Screen Actors Guild from 1985 to 1988, presiding over the 22nd Screen Actors Guild Awards.
