Medical Training and Practice
Palacio completed his primary education at the Abdón Calderón and San José La Salle schools before earning a PhD in Medicine and Surgery from the University of Guayaquil in 1967. He expanded his expertise in the United States, undertaking medical residency at Case Western Reserve University and completing a cardiology fellowship at Barnes-Jewish Hospital. In 1980, he founded the Instituto Nacional de Cardiología Alfredo Palacio in his home city, where he treated notable figures including León Febres-Cordero and Sixto Durán-Ballén.

In 1994, President Sixto Durán-Ballén appointed Palacio as the Minister of Public Health. During his tenure, which lasted until 1996, he addressed significant national health crises, including outbreaks of cholera, malaria, and chicken pox. He was also tasked with organizing emergency medical support during the 1995 Cenepa War, an effort that earned him recognition from the Ecuadorian Armed Forces. His policy stance focused on opposing the privatization of the national health infrastructure.
The Vice Presidency and Political Rupture
Palacio joined the electoral ticket of the Patriotic Society Party as the running mate for Lucio Gutiérrez in the 2002 general election. Following their victory, he was sworn in as Vice President of Ecuador in January 2003. Friction between the two leaders quickly developed, fueled by policy disputes and disagreements over the government’s alignment. By 2004, Palacio publicly distanced himself from the administration’s economic and social trajectory, leading to a breakdown in their professional relationship.
Transition to Presidency
The administration faced a terminal crisis in 2005 following the controversial restructuring of the Supreme Court, a move characterized by critics as an authoritarian overreach. As mass protests engulfed Quito, Palacio publicly challenged the government's measures. After the Ecuadorian Congress removed Gutiérrez from office in 2005, Palacio assumed the presidency. He served in this capacity until 2007, maintaining a focus on his progressive platform before leaving office.
