Werner Forssmann: The Doctor Who Catheterized His Own Heart

In 1929, a twenty-five-year-old surgical resident in Eberswalde persuaded a nurse to help him attempt a procedure his superiors had explicitly forbidden — then, once she was on the operating table believing she would be the patient, quietly restrained her and performed it on himself instead. Werner Forssmann threaded a urinary catheter into a vein in his own arm and, guided by a fluoroscope, walked down the hall to the X-ray room to watch it travel sixty centimeters into his own beating heart.

A Berlin Childhood Cut Short by War

Forssmann was born on August 29, 1904, in Berlin, in the German Empire. He lost his father, a lawyer, during the First World War, and completed his secondary education at the Askanisches Gymnasium in 1922 before enrolling in medical school at Friedrich-Wilhelm University in Berlin. He passed his state medical examination around 1928–1929 and began a surgical residency at the Auguste-Victoria-Heim hospital in Brandenburg, working under a senior physician, Dr. Schneider, who explicitly forbade him from pursuing an idea he had become fixated on: threading a catheter directly into the human heart.

The Self-Experiment

Cardiac catheterization had been attempted only in animals at the time; the medical consensus held that introducing a foreign object into a beating human heart would trigger fatal cardiac arrest. Forssmann believed otherwise, and when denied permission to test his idea on a patient, he recruited an operating-room nurse, Gerda Ditzen, to assist him — securing her cooperation, according to multiple accounts, under the understanding that she herself would be the subject. Once she was restrained on the operating table, Forssmann instead performed the procedure on himself: under local anesthesia, he inserted a ureteric catheter into the antecubital vein of his own arm, advanced it first roughly thirty centimeters and then, on a further attempt, sixty centimeters, and walked to the hospital's fluoroscopy room to confirm by X-ray that the catheter tip had reached his own right ventricle. He documented the result on X-ray film and published the account, "Die Sondierung des rechten Herzens" ("The Probing of the Right Side of the Heart"), in the journal Klinische Wochenschrift on November 5, 1929. The paper triggered immediate press attention and professional backlash: senior surgeon Ferdinand Sauerbruch effectively forced Forssmann out of Berlin's Charité hospital over the stunt, and the young physician's cardiology ambitions were, for the moment, over.

A Career Redirected

Cut off from cardiology, Forssmann shifted into urology, working at institutions including the Städtisches Krankenhaus in Mainz and later the Rudolf-Virchow-Krankenhaus. In 1933 he married Dr. Elsbet Engel, herself a urology specialist, and the couple would go on to have six children, several of whom followed him into medicine — most notably Wolf Forssmann, who later helped isolate atrial natriuretic peptide, and Bernd Forssmann, who contributed to developing the first clinical lithotriptor for treating kidney stones. Forssmann joined the Nazi Party in 1932 and remained a member through 1945, and during the Second World War he served as a medical officer in the Wehrmacht, rising to the rank of major before being captured and held in an American prisoner-of-war camp. Released in 1945, he found his wartime service and party membership blocked him from resuming ordinary medical practice for several years; he supported his family by working as a lumberjack in the Black Forest until French occupation authorities permitted him to return to country medical practice around 1948, and he opened a urology practice in Bad Kreuznach in 1950.

Recognition, Two Decades Later

Forssmann's self-experiment might have remained a forgotten medical curiosity had two other researchers, André Cournand and Dickinson Richards at Columbia University in New York, not built on his technique through the 1930s and 1940s, refining cardiac catheterization into a rigorous diagnostic tool for measuring pressures and blood flow inside the living heart. In 1956, the Nobel Prize in Physiology or Medicine was awarded jointly to Forssmann, Cournand, and Richards for their discoveries concerning heart catheterization and its diagnostic use in disorders of the circulatory system — vindication, decades late, for the procedure that had once cost Forssmann his career. He had already received Germany's Leibniz Medal in 1954. In the years following his Nobel recognition, he became chief of surgery at the Evangelisches Krankenhaus in Düsseldorf by 1957, and was later named an honorary professor of surgery and urology at the University of Mainz as well as an honorary professor at the National University of Córdoba in Argentina in 1961, alongside memberships in numerous international medical societies.

Why Werner Is Called a Genius

Forssmann's case for genius does not rest on abstract theoretical insight; it rests on an act of physical daring combined with a refusal to accept an unproven medical taboo. The prevailing wisdom of 1929 held that a foreign object in the heart would kill a patient outright, and no controlled evidence yet existed to contradict that belief in a living human being. Forssmann's contribution was to test the boundary himself rather than wait for someone else to risk it, and to have the physiological self-possession to walk, catheter in his own vein, to an X-ray suite to document what he had done. That is a genius of nerve and conviction more than of intellect in the traditional sense — the underlying medical reasoning was sound but not itself revolutionary, and it took Cournand and Richards's subsequent, more systematic laboratory work to turn Forssmann's stunt into a genuine diagnostic method, which is precisely why the Nobel committee split the prize three ways. The complicating shadow over any celebration of Forssmann is his own conduct: he deceived a colleague to gain access to the operating table, and he was a member of the Nazi Party through the war years, facts that sit uneasily alongside the audacity that made him a Nobel laureate.

Legacy

Forssmann died on June 1, 1979, in Schopfheim, West Germany, of heart failure, at age seventy-four. The technique he pioneered on himself became the foundation of interventional cardiology, opening the door to modern procedures from coronary angiography to the Swan-Ganz catheter used today in intensive care units worldwide — a direct descendant of the sixty-centimeter tube a disgraced young resident once guided into his own right ventricle to prove a point his superiors would not let him test any other way.
