He graduated from Wakefield High School in 1963. He pursued pre-med studies at George Washington University before entering the Medical College of Virginia at Virginia Commonwealth University, where he earned his Doctor of Medicine degree in 1971.

After graduating, Adams and his associates initiated a communal home environment known as the Zanies, which operated from 1971 to 1984. This project served as the precursor to the Gesundheit! Institute. The organization maintains a property in rural West Virginia intended for the construction of a 44-bed community hospital. This facility aims to provide free holistic care alongside a teaching center designed to accommodate 120 staff members within a communal ecovillage.
Activism and Global Outreach
Adams bridges medicine and social activism, leading global volunteer clowning missions to areas affected by natural disasters, such as Sri Lanka after the tsunami. He serves as an honorary chair of the International Association for the Advancement of Creative Maladjustment. Additionally, he supports the Ithaca Health Alliance and its free clinic, aligning with his advocacy for ending capitalism within medical structures.

The 1998 biographical film "Patch Adams" portrayed his medical philosophy, though Adams criticized the production for simplifying his activist beliefs and focusing excessively on the comedic aspects of his practice.
