By 1989, he secured election to Dáil Éireann for the Cork South-Central constituency, a seat he has held consistently through subsequent electoral cycles, including the 2002–2007, 2016–2020, 2020–2024, and 2024 terms. In 2000, he moved to the Department of Health and Children, where he established the Health Service Executive and implemented the public smoking ban. His cabinet trajectory continued with a tenure as Minister for Enterprise, Trade and Employment from 2004 to 2008.
