Early Career and Domestic Development
Born in Sydney in 1985, Milligan attended Trinity Catholic College and began his association with the sport at Birrong Sports Football club in 1991. After progressing through junior ranks, he joined Sydney FC. In 2008, he had a brief loan spell with the Newcastle United Jets. His international ambitions led to reported interest from European clubs, including Blackburn Rovers, Manchester City, and Arsenal, though he eventually sought opportunities in the Asian market.

Milligan joined Shanghai Shenhua in January 2009, scoring his first goal for the club during an AFC Champions League match against Kashima Antlers. He later moved to JEF United Chiba in Japan. Following the 2011 Tōhoku earthquake and tsunami, he temporarily returned to Sydney FC to maintain his fitness while the J2 League was suspended, eventually departing Japan in 2012.
Melbourne Victory Stints
In 2012, Milligan joined Melbourne Victory, becoming the first player to move from Sydney FC to the club. He excelled in a defensive midfield role during the 2012–13 season, earning the Victory Medal. Appointed captain in 2013, he led the team to a 3–0 win over Sydney FC in the 2014–15 A-League Grand Final, receiving the Joe Marston Medal. He returned to the club for a second stint as a marquee player in 2017.
Global Club Assignments and Retirement
Milligan played for Baniyas SC in the UAE, Al-Ahli in Saudi Arabia, Hibernian in Scotland, and Southend United in England. In 2020, he returned to Australia to join the newly formed Macarthur FC as their inaugural captain. He scored the club's first goal in a match against Western Sydney Wanderers. Milligan retired from professional play at the conclusion of the 2020–21 season to join the Macarthur coaching staff and currently serves as head coach of the Newcastle Jets.
