Ferdinand von Hebra: The Man Who Made Skin a Science

In 1844, a young Viennese physician settled an argument that had run for more than a century by the simple method of looking. Scabies, the medical establishment held, was a disorder of the humours — a corrupt fluid working its way out through the skin, with the burrowing mite an incidental passenger if it existed at all. Ferdinand Hebra demonstrated that the mite was the disease. It was a small finding with an enormous implication: skin conditions might have local, physical, findable causes. The whole of modern dermatology follows from taking that thought seriously.

Brno to the Vienna General

Hebra was born Ferdinand Karl Franz Schwarzmann on 7 September 1816 in Brno, in Moravia, then part of the Austrian Empire. He began his studies in Graz and graduated in medicine from the University of Vienna in 1841, arriving at exactly the right institution at exactly the right moment.

Vienna in the 1840s was reinventing medicine. Carl von Rokitansky was building pathological anatomy into a systematic discipline in the autopsy room; Joseph Škoda was doing the equivalent at the bedside with percussion and auscultation. Together with Hebra they formed what Viennese medicine called the *Dreigestirn*, the triple star, and between them they hauled the city back to the front rank of European medicine. Rokitansky's influence on Hebra was decisive and specific: Hebra learned from him that a disease is defined by the structural change in tissue that produces it.

He was given the ward nobody wanted. Chronic skin patients — the scabies cases, the incurably itching — were the dregs of hospital assignment, warehoused rather than treated. Hebra took the assignment and turned it into a laboratory. In 1849 he became the first professor of dermatology in the German-speaking world at the Vienna General Hospital.

The Classification of 1845

His foundational act was an act of sorting. In 1845 Hebra published a classification of skin diseases organised by pathological anatomy — by what was structurally happening in the tissue — rather than by surface appearance or by supposed constitutional cause. It was the direct application of Rokitansky's method to the body's largest organ, and it held the field for close to a century.

The achievement is easy to underrate now because the alternative has vanished from memory. Before Hebra, skin diseases were named for how they looked and explained by whatever humour was thought to be out of balance; two conditions with entirely different mechanisms would be filed together because they were both red and scaly, while the same disease at two stages would be filed apart. Hebra's system made dermatology a subject in which knowledge could accumulate, because for the first time two physicians in different cities could be confident they were discussing the same thing.

He used the new clarity to describe and name a great deal of what he had sorted — lupus, pityriasis rubra, tinea cruris among them — and he was the first to describe dermatitis herpetiformis. In the later part of the century he also pioneered the therapeutic use of chemical agents on the skin, employing phenol, croton oil and nitric acid in what were, in effect, the first chemical peels.

The Atlas

In 1856 Hebra began publishing the *Atlas der Hautkrankheiten*, and it is arguably the most consequential thing he ever made. Skin disease is a visual subject; a classification is useless if practitioners cannot recognise the categories. The atlas solved that with life-sized coloured illustrations produced by two Viennese painter-physicians, Anton Elfinger and Carl Heitzmann — men who could both draw and diagnose. It ran to a tenth edition by 1876 and has been called one of the most influential dermatological texts of all time.

He began the accompanying *Lehrbuch der Hautkrankheiten* in 1878 but did not live to finish it; his student Moritz Kaposi completed it. Translated widely, it served for decades as what colleagues simply called the Bible of dermatology.

The School

What converted Hebra's work from a personal achievement into an institution was teaching. Vienna became the place physicians came from everywhere to learn skin disease, and his students carried the method outward. Moritz Kaposi — who described the sarcoma that bears his name — became his successor and his son-in-law. Heinrich Auspitz and Isidor Neumann were also his pupils, and the German dermatologist Paul Gerson Unna trained in Vienna under Hebra, Kaposi and Auspitz before founding the discipline of dermatopathology in Hamburg. The comparison contemporaries reached for was Albrecht von Graefe's position in ophthalmology: one man who converted a neglected corner of medicine into a specialty with its own science.

Semmelweis

One episode complicates the portrait usefully. Hebra was an early and public supporter of Ignaz Semmelweis, announcing the handwashing discovery in medical journals in December 1847 and again in April 1848, when almost nobody in Vienna would. He remained the only friend who kept in touch with Semmelweis after he left the city. He also took part in arranging Semmelweis's committal to an asylum in 1865, where Semmelweis died within a fortnight. Both facts are true, and neither cancels the other.

Why Ferdinand Is Called a Genius

Hebra has been called the undisputed potentate of *Hautkrankheiten* and, without much argument, by far the most important dermatologist of the nineteenth century. The quality at issue is not theoretical originality — he discovered no mechanism and proposed no new physiology. It is taxonomic judgement: the ability to look at a chaotic mass of appearances and see which distinctions carry information and which are noise. That is a rarer talent than it sounds, and it is the one that determines whether a field can develop at all. Hebra also had the specific courage to refuse a comfortable universal explanation. Humoral theory could account for anything, which is precisely why it accounted for nothing; Hebra gave it up in favour of local causes he could actually demonstrate, starting with a mite.

The honest counter-case is that much of what Hebra did was imported. Rokitansky supplied the method; Hebra applied it to a new organ. His insistence on local causation, so liberating in 1845, hardened into a dogma that his school defended past its usefulness — many skin diseases really are systemic, and constitutional and immunological explanations that later proved correct met resistance in Vienna partly because of him. His atlas depended on artists whose contribution was indispensable and is rarely credited. A sceptic could reasonably file him as an outstanding organiser, clinician and teacher rather than a discoverer. The reply is that organising a subject into a form where discovery becomes possible is itself a creative act, and one that fewer people can perform than can make discoveries within an established frame.

Legacy

Hebra was ennobled as Ritter von Hebra. In 1878 he was elected president of the College of Physicians in Vienna, but he died before chairing a single meeting — on 5 August 1880, in Vienna, of pulmonary emphysema, aged sixty-three.

His ward is gone, his classification has been superseded, and his textbook is a historical curiosity. What survives is the assumption underneath all of it: that a rash is a physical event in tissue, that it can be classified by what is happening rather than by what it resembles, and that a physician should look before theorising. Every dermatology clinic in the world runs on Hebra's premise without noticing it is his.
