Ambroise Paré: The Surgeon Who Ran Out of Oil

At the siege of Turin, a barber-surgeon in his mid-twenties reached for the boiling oil that every authority said must be poured into gunshot wounds, and found the supply exhausted. He improvised a dressing of egg yolk, rose oil and turpentine. He did not sleep that night, expecting to find the untreated men dead by morning. Instead he found them, in his own words, "feeling little pain from their wounds which were not swollen or inflamed," while the men who had received the oil lay "fevered, with great pain and swelling." Modern surgery begins somewhere near that morning.

A Trade, Not a Profession

Ambroise Paré was born around 1510 at Bourg-Hersent, near Laval. He entered medicine through its back door. Surgery in sixteenth-century France was a craft, practised by barber-surgeons who cut hair, drew blood and amputated limbs, and who ranked far below the university physicians who read Galen in Latin and rarely touched a patient. Paré apprenticed under his barber-surgeon brother in Paris and trained at the Hôtel-Dieu, France's oldest hospital — an education of hands and cases rather than texts. He never mastered Latin. He would write in French, which was itself a provocation: it put surgical knowledge in reach of men the faculty did not consider colleagues.

Turin, and the Argument Against Fire

The doctrine Paré inherited came from Giovanni da Vigo, who held that gunshot wounds were poisoned by gunpowder and must therefore be cauterised — filled with oil heated to boiling. The treatment produced burns on top of injuries, and the agony was legendary.

Paré's accident at Turin in 1536–37, during the Piedmont campaign, gave him a comparison group he had not asked for. What he did with it is the point. He did not merely note that his improvised balm worked better; he attacked the theory underneath the practice. Gunpowder, he reasoned, is sulphur, charcoal and saltpetre, and these "are not poisonous separately and so they can't be poisonous as a mixture." No poison, no need for fire. He added an argument that was moral rather than physiological: a Christian should not inflict avoidable suffering before trying the gentler remedy first. Reducing the patient's pain during treatment was, at the time, an almost unprecedented principle to place at the centre of surgery.

He published the results in *The Method of Curing Wounds Caused by Harquebus and Firearms* in 1545, and returned to them across a long publishing career culminating in the collected *Oeuvres*, which appeared in 1575 and again in 1585 and was reprinted internationally for generations.

The Ligature

His second great intervention concerned amputation. The standard method of stopping haemorrhage from a severed limb was, again, the cautery — a red-hot iron pressed to the stump. Paré reintroduced the ligature: tying off the individual arteries with thread. To do it he needed an instrument that could seize a retracted vessel in a bleeding wound, and he designed one, the *bec de corbeau* or crow's beak. The ligature was slower than the iron, demanded far more anatomical precision, and in an era without antisepsis it carried its own risk of introducing infection. But it was the technique that survived, and it is the ancestor of every vascular clamp in a modern operating theatre.

Beyond the Battlefield

Paré's curiosity did not stop at wounds. He revived podalic version in obstetrics — turning a malpresenting foetus in the womb to deliver it feet first — a manoeuvre that had been effectively lost, and he influenced the training of the royal midwife Louise Bourgeois. He designed prosthetic limbs and made artificial eyes from gold, silver, porcelain and glass. He described phantom limb pain and, with a startling leap, located its origin in the brain rather than in the residual tissue of the stump, which is where the modern account puts it. His writing on the effects of violent death on the internal organs, and on how a surgeon should report medical findings to a court, laid groundwork for forensic pathology.

He served as surgeon to four French kings — Henry II, Francis II, Charles IX and Henry III — through the Italian Wars and into the Wars of Religion, and he attended Henry II's fatal jousting injury. He was a working military surgeon for most of the period between 1542 and the 1580s, which is to say he spent his life in the one setting that produces trauma faster than theory can absorb it.

Why Ambroise Is Called a Genius

Paré's claim rests on craft and temperament rather than on intellect in the scholar's sense, and it is more impressive for that. He read no Galen in the original, proposed no system, and made his central discovery by running out of a supply. What he possessed was a capacity that the learned medicine of his century almost entirely lacked: he trusted what he had seen in front of him over what the authorities had written, and he then reasoned from the observation back to the theory and demolished it. The gunpowder argument — three harmless substances cannot combine into a poison — is a genuine piece of reasoning, not a lucky accident. So is the inference from a phantom limb to the brain.

The counter-case is real and was made in his lifetime. His empirical claims were contested: the physician Julien Le Paulmier argued that only a few of Paré's battlefield patients survived, and those were not seriously wounded. Mortality figures and case severity for his campaigns cannot be reconstructed, so no confident statistical claim about his results is possible. Da Vigo's cauterisation did not vanish; his textbook was still in demand in the eighteenth century. The ligature took two centuries to become routine. Paré changed practice more slowly than the legend suggests, and he was as capable as anyone of his age of believing things — about monsters, prodigies and unicorn horn — that were simply wrong. His genius was not omniscience. It was the willingness, rare and repeatable, to let a case overrule a book.

Legacy

The line most often quoted of him is the humblest thing he wrote: *Je le pansai, Dieu le guérit* — "I dressed him and God healed him." It is not false modesty so much as an accurate division of labour as he understood it, and it doubles as a description of his method: do the least harm the wound allows and let the body do what it can. He died in Paris on 20 December 1590, around eighty years old, having raised a barber's trade into something that could stand beside medicine and, in the operating theatre, eventually outgrow it.
