Early Development and Italian Beginnings
Appiah began his path in professional football in 1995 with the Ghanaian club Hearts of Oak at age 15. By 1997, he secured a move to the Italian Serie A, joining Udinese. Although he initially played as a striker, he transitioned into a deeper midfield role during his three-season tenure. After a transfer to Parma was delayed due to a bout of viral hepatitis, he eventually joined the club in 2000. He later joined Brescia on loan for the 2002–03 season, where he established himself as a first-team regular, scoring seven goals in 31 matches.

In the summer of 2003, Juventus signed Appiah on loan from Parma for €2 million, with an option for a permanent transfer. He appeared in 30 Serie A games and played in the Coppa Italia final during his first season, while also making his debut in the UEFA Champions League. In 2005, he moved to the Turkish club Fenerbahçe for €8 million, helping the team secure the Süper Lig championship during their centenary year.
Contractual Disputes and Legal Resolution
A severe knee injury in 2007 led to a complex dispute between Appiah and Fenerbahçe. After the player delayed surgery and refused to undergo rehabilitation in Turkey, the club attempted to de-register him. Appiah unilaterally terminated his contract in 2008, alleging mistreatment.
Later Career and Final Matches
Following unsuccessful trials at Tottenham Hotspur and Rubin Kazan, Appiah returned to Italy, signing with Bologna in 2009 and later with Cesena in 2010. In February 2012, he joined the Serbian club Vojvodina. He scored his only goal for the team during a 2-1 victory over Red Star Belgrade, helping secure a spot in the UEFA Europa League. Following two years of inactivity due to recurring knee issues, he announced his retirement on 14 January 2015 at age 33. A testimonial match at Accra Sports Stadium on 27 June 2015 marked his departure from the sport.
