Clinical Work in Afghanistan
Hamilton arrived in Kabul in 1894 after being invited by the Emir, Abdur Rahman. She served as his personal physician for three years and established a hospital in the region. Her medical contributions included the introduction of vaccination protocols to the Afghan population, while she also navigated the local medical landscape by incorporating traditional beliefs regarding the body's four humors.

Beyond her clinical practice, she was an author who documented her observations. She published 'A Vizier’s Daughter: A Tale of the Hazara War' in 1900, a work that blended fictional narratives with critical perspectives on local power structures. She also expressed reservations regarding the political reforms of the Emir. Later, while residing in England, she joined the Women’s Freedom League to support the movement for voting rights.
Educational Leadership and Wartime Service
In 1908, Hamilton took the role of warden at Studley Horticultural College in Warwick, an institution focused on training women for agricultural careers. Her tenure was interrupted in 1915 when she volunteered for the Wounded Allies Relief Committee. She managed a hospital in Podgoritza, Montenegro, during the war, before returning to her duties at Studley until her retirement in 1924.
