Early Career and Israeli Stints
Paintsil began his professional path in Ghana with Berekum Arsenal and Liberty Professionals. Following a brief period with the Polish team Widzew Łódź, he moved to Maccabi Tel Aviv in 2002. During his time there, he contributed to the team winning the Israeli title and reaching the Cup semi-final. In December 2004, he transferred to Hapoel Tel Aviv, where he secured an Israeli State Cup victory in 2006.

In August 2006, West Ham United signed Paintsil after a recommendation from Yossi Benayoun. He wore the number 14 shirt during his two-year stay in London. In July 2008, he transferred to Fulham as part of a £6.3 million deal alongside Bobby Zamora. He became a first-team regular, contributing to the club's seventh-place finish in 2009. His subsequent Premier League stint at Leicester City in 2011 was brief, ending in 2012.
International Representation
Paintsil represented Ghana extensively, including the 2001 FIFA World Youth Championship where the team finished as runners-up. He participated in the 2002 and 2006 African Cup of Nations, as well as the 2004 Olympics. During the 2006 World Cup, he appeared in every match for Ghana. He later played in all five of Ghana's games during the 2010 World Cup, where the team advanced to the quarter-finals.
Coaching and Recent Activity
Following his retirement from professional play on 24 June 2016, Paintsil transitioned into coaching. He served as an assistant manager at Kaizer Chiefs starting in June 2016. Currently, he works as an assistant coach for the Ghana national team, the Black Stars, assisting with preparations for the 2026 FIFA World Cup. In September 2024, he survived a vehicle accident while traveling with Otto Addo and Fatau Dauda.
