Jacques Guillemeau: The Surgeon Who Counted a Hundred and Thirteen
The title page of his first great book is a small act of accounting: *Traité des maladies de l'oeil, qui sont en nombre de cent treize* — a treatise on the diseases of the eye, which are one hundred and thirteen in number. Not "many." Not "various." One hundred and thirteen, enumerated, each with its signs and its remedy. It was the first French work on ophthalmology, written by a man in his mid-thirties who had spent eight years living in Ambroise Paré's house, and its precision is the signature of everything he did afterwards.
Eight Years in Paré's House
Guillemeau was born in Orléans in 1550, into a family of surgeons — an important detail in a century when surgery was still a craft learned by apprenticeship rather than a subject taught at university. Physicians read Galen in Latin; surgeons cut, and were socially beneath them.
He was placed with Ambroise Paré, the greatest surgeon in Europe and the man who had abolished the practice of pouring boiling oil into gunshot wounds. Guillemeau lived in his household for eight years, an immersion rather than a course of study, and the relationship became familial in the literal sense: he married Paré's daughter and became his son-in-law as well as his pupil.
He never stopped acknowledging the debt. The dedication of the eye treatise, addressed to Paré as counsellor and first surgeon to the King, is unusually naked for a Renaissance dedication: "I would wish to make known to everyone how much I owed you for having been instructed in your house for eight years."
Flanders
Surgery in the sixteenth century was learned on battlefields, because battlefields were where the wounds were. Guillemeau accompanied Paré on military campaigns and then spent four years serving with the Spanish army in Flanders — the long, grinding war in the Low Countries that produced amputations, gunshot fractures and infected wounds in industrial quantity.
This is where the hand was trained. A surgeon who has done a hundred field amputations under fire develops a relationship with anatomy and with speed that no amount of reading supplies, and it explains why Guillemeau's later books are so relentlessly practical. He came back to Paris in 1581 and established himself, working at the Hôtel-Dieu, the great charity hospital on the Île de la Cité.
The Hundred and Thirteen
The *Traité des maladies de l'oeil* appeared in 1584 or 1585 — the sources differ by a year — and it remains one of the finest ophthalmic works of the Renaissance. Its method is the enumeration: take an organ, exhaust its pathology, describe each condition in turn. Within it Guillemeau gave the first account of the repair of a palpebral coloboma, the congenital notch or cleft in the eyelid, a procedure he called *paupières accourcies*, shortened eyelids. He also left a description of ophthalmoplegia — paralysis of the eye muscles — that modern ophthalmologists have gone back to read.
The book crossed the Channel quickly. An English translation appeared in 1587 as *A worthy treatise of the eyes*, which tells you something about how fast a genuinely useful surgical text travelled in a Europe otherwise divided by war and religion.
Surgeon to Three Kings
He became surgeon-in-ordinary to Charles IX, to Henry III, and to Henri IV — three monarchs across the worst decades of the French Wars of Religion, two of whom were assassinated. Serving all three required not merely skill but the political sense to be indispensable to whoever was on the throne.
The books kept coming. *Tables anatomiques* in 1586 laid out the skeletal, digestive, circulatory and nervous systems with illustrations. *La Chirurgie française* in 1594 was an illustrated catalogue of surgical instruments with descriptions of their use — a manual, not a treatise, aimed at the hands of working surgeons. His collected works were gathered in 1612, the year before he died.
The Delivery Room
The most consequential of his books was the last major one. *De l'heureux accouchement des femmes* — the happy delivery of women — appeared in 1609, and in it Guillemeau did for obstetrics what he had done for the eye.
Two contributions stand out. He gave the first description of assisted breech delivery: what a birth attendant should actually do with their hands when a child presents feet or buttocks first, which until then had been very close to a death sentence for both mother and infant. The manoeuvre he set down is the one later attached to other names, popularised as the Mauriceau-Smellie-Veit manoeuvre after men who came decades and centuries later. He also practised podalic version — turning the child inside the womb to bring the feet down — in cases of placenta praevia, where the placenta blocks the birth canal and the mother bleeds.
An English translation followed in 1612. In the same year as the French original he published *De la nourriture et gouvernement des enfants*, on the feeding and care of infants, making him one of the founders of paediatrics as a written subject as well as of obstetrics.
Why Jacques Is Called a Genius
No source calls Guillemeau a genius, and the word sits awkwardly on him. He was Paré's pupil and worked inside Paré's framework; he founded no theory; his anatomy was inherited. Much of his reputation is proximity — the son-in-law of the great man, faithfully extending the great man's method.
What the record actually shows is a specific and undervalued form of intelligence: systematisation. Guillemeau's gift was to take a domain that existed only as scattered practical lore in the hands of individual craftsmen and convert it into an ordered, complete, transmissible text. One hundred and thirteen diseases of the eye is not a rhetorical flourish; it is a claim to have covered the field. The instrument catalogue, the anatomical tables, the step-by-step account of what to do with a breech presentation — all of it is the same operation performed on different material. He was turning craft into knowledge, which is the move that lets a discipline accumulate rather than repeat itself.
The honest counter-case is that this is editorial and pedagogical skill rather than discovery, and that the manoeuvre bearing three other men's names is a fair verdict on how much of it was truly his invention rather than his description of common practice. He was also, plainly, a man of extraordinary manual courage — four years of Flemish battlefields, then a career of operating on eyes and delivering obstructed labours without anaesthesia, antisepsis or the possibility of a second attempt. If genius here means anything, it means the nerve to work at that scale of consequence combined with the discipline to write down exactly what he did.
Legacy
Guillemeau died in 1612 or 1613, having founded three specialties in French: ophthalmology, obstetrics and the care of infants. His son Charles continued in medicine. The manoeuvre he first described is still performed in delivery rooms, under other names; the palpebral repair he devised is still the basic logic of eyelid reconstruction.
His deeper inheritance is the format. Before Guillemeau, surgical knowledge lived in households like Paré's and died with the men who held it. He built the alternative — the enumerated, illustrated, translated manual that any competent practitioner in any city could open and use. It is why a Frenchman's account of the eye was in English within three years, and why a technique worked out in Paris in 1609 outlived every king he served.
